Natural birth after 4th degree tear?

Hi, this is my second pregnancy and i had a pretty awful labor and delivery with my first. After 42 hrs labor and 2hrs pushing my daughter was born with assistance which caused me to tear quite badly. My midwife tells me that I won't have any problem this time around but I have since been told that midwives here get a cash bonus for natural births. I just need some impartial advice before I decide on the birth plan. I don't want to have another tear like that again!

I was the same way with my first n my mom was also and she had 3 kids how ever it makes the other deliveries easier I'm preggo with my second as well I think we should be fine

Thanks. I talked to my dr and he says I should be OK but would love to hear from someone who has been through something similar.
