Every carseat has this issue... Make sure the cover is on properly and that the child is not reaching under the cover to grab the foam. If the child is properly secured and the cover is on properly they shouldnt be able to reach it. We had a Britex Boulevard with our daughter so we experiences this also... its a voluntary recall too...
No the chest pads are made of a rubbery material and kids (including my son) are biting pieces of the rubber off and getting choked. Carson hasnt choked on it but he had it tore up the very first day he sat in it from chewing on it
