**Need Support/Advice: 2nd Rate Pre-natal Care**

First, sorry this is SO long! I'm 31 years old, & pregnant for the 3rd time, at 25 weeks now. I was laid off from my job in August, had lap-band surgery in September, & got pregnant while using condoms & not ttc just 19 days after surgery. When my severance ran out from my job, I had to go on state health insurance, called Access Plus, after having lost Blue Cross/Blue Shield coverage. The ONLY place that accepts the state insurance & high-risk patients is the Conemaugh Women's Clinic. But the care there is horrible. I have 2 close friends & a cousin who miscarried while under their care because they didn't treat low Progesterone issues in time. I'm considered high-risk due to multiple miscarriages, hypothyroidism, PCOS, & blood clotting problems, yet they have never measured my belly, don't test my urine, & refuse to give me ultrasounds other than my 10 week, 20 week, & they plan to do one more at week 32. I'm terrified that something could be going wrong or will, & they'll miss it by not checking on her more than just 3 times in 9 months of pregnancy.

I've never made it past week 8 in a pregnancy prior to this one.

The most frustrating part is that when I saw my surgeon, he said lap-band babies need to be monitored for growth every 4-6 weeks because we can't consume the same amount of nutrition as normal mom's. Even with my band fully open, I can't eat much and have only gained 2.5 pounds at 6 months & 1 week pregnant. I told my OB (I see a different doc each time, & have to explain my history over & over again cause they are too busy to read your chart before seeing you) & she blew it off like I was talking nonsense. I said for her to please consult with my surgeon, & she looked at me as if I were stupid & said, "I will, if I think it's necessary."

This place never tells you test results & when I had my 20 week ultrasound, I had to literally grab the OB by her arm as she tried to rush out the door & ask if everything was normal. She brusquely flipped through my chart & said, "The tech would have left me a note, so I think you're fine." Then just left the exam room.

I called Access to see if there was anywhere else at all I could go, & I even called Conemaugh Hospital's patient relations department. For both, I was told that the clinic is my ONLY choice for pre-natal care...mainly due to the insurance I have. This just kills me, because I know most high-risk momma's out there are receiving good care for themselves & their babies. (I pray so!) I try not to stress, but I am SO frightened. I love my daughter with all my heart & just want her to be healthy & go as close to full-term as possible. But I'm trapped in a clinic that sees an average of 100 patients per day with a staff of two OB's & 3 nurses, always rotating that staff, & their main focus is to get you in & out as quickly as possible. Treating you like a number, & not a person, in the process. Again, I'm sorry this is so long, but I'm really scared & needed to talk about it with other women who would understand my fears & concerns.

    is this state insurance? here in texas we have medicaid and im wondering if its similar to that? ok well im going to assume it is and in that case yes you are limited as far as ob offices because only certain ones take that insurance carrier but an option might be to travel to a nearby town to get your care at a facility that accepts your insurance there? i know with the lap band it makes pregnancy much riskier especially so soon after but cant you have your lap band loosened if need be? and as far as weight gain that depends totally on your beginning weight- since you had just had weight loss surgery im going to assume you were over weight and in that case 10-15 lbs is all thats recomended but itd be extra important for you to be taking prenatals and eating vitamin rich foods to ensure what does get absorbed is good for baby. how much weight have you lost since your surgery or have you only gained from your pregnancy? oh and if worst comes to worst go to the hospital- it may not be an emergency but if its state insurance theyll cover all those er visits and if you feel somethings wrong at least theyll check you out and give you a ultrasound.
    im on texas state health insurance this time around because my s/o's insurance hasnt approved to take me on and may not since i was pregnant slightly before the sign up date, but so far my care is excellent ive got the same dr i had with ds and anytime i have a concern they do whatever is needed and dont seem to be penny pinching,im so sorry your experience is so terrible.
  • What state do you live in? I don't know much about state funded health care besides my own (California) but here we have medi-cal and there are a huge amount of private dr's who accept it. Do you have a case worker or anyone you can call and see about getting seen somewhere else?
  • @carolyntx: Yes, it's state insurance. I'm not familiar with Medicaid but have heard ladies mention it on here before. The only reason I qualify for Access is because I lost my good insurance from being laid off, am pregnant & was uninsured, & I'm making over $400 less a month on unemployment than when I was working. It's like welfare, low income insurance - one of a few plans Pennsylvania offers. I'm grateful to even have coverage, don't get me wrong, but I wish I could have found a job that offered good medical insurance by now. I'm to the point where I really can't work anywhere cause of all my restrictions on activity.
  • do you still have the option of picking up cobra insurance from bluecross blueshield? since your on unemployment they should help cover the expense-- thanks obama lol-- you should check into if your dates have expired
  • @misskerribabee: I live in Pennsylvania, & unfortunately, in my area, only two places accept Access. The clinic I'm trapped at now, and Windber Hospital's OB group right in my town. However, Windber Obstetrics will not take high-risk cases, & since I'm labeled as such, they turned me down back in December, saying I was better off at Conemaugh. If they only knew how wrong they were! My cousin's fiance had her baby with Windber, & when we compare notes on the care we both have had, it's sad & startling how much better hers was. And she wasn't high-risk. So frustrating! :*(
  • @carolyntx: My Cobra has expired, but the price I was given was $342 a month. After losing $400 a month in income on UC, that was impossible for me to afford. I own my home & have a mortgage, utilities, etc. $342 a month for Cobra continuation is more than my mortgage! :(
    I was like, really? How am I supposed to do this? And the most frustrating part is having lost my job due to lack of work, getting pregnant at the worst possible time ever, & now I feel absolutely trapped getting crappy pre-natal care with no hope of finding better for me & my baby. Ugh!!
  • @carolyntx: I forgot to answer your other questions above. Yes, I started this pregnancy being overweight & was told I should only gain 12-15 lbs. altogether. Between the pre-surgery dieting & the 19 days I wasn't pregnant afterwards, I had lost around 40 lbs. & have thus far gained back 2.5 lbs. I take my pre-natals religiously, & always try to eat the best foods possible, but a lot of stuff just won't make it down through the band, even though its been opened as wide as it can go.
    I've thought about going to the ER either this week or next, just to have that peace of mind of seeing her again. I really think I might! Thank you so much for caring! :)
  • I would really look in to medicaid. I have it! plus with my medicaid card came a website where i can look up all kinds of doctors and hospitals that can tae care of ur kind of pregnancy. It really helps.
  • @2ndbby_ondaway: I will do some internet research to see if Pennsylvania has Medicaid. It sounds WAY better than Access, but I'm totally unfamiliar with these programs. I was always lucky to have great medical insurance my whole life; first, through my father, then my ex-hubby, & then through my own job. I have an amazing family physician, & my lap-band surgeon is literally award-winning & highly sought after. So dealing with such sub-standard medical care is difficult & unacceptable to me. And really, no pregnant woman, high-risk or not, should have to deal with sh*tty treatment from their OB. We need them to listen & support us, & treat us like human beings, not an assembly line production - get em in, out, & on their way. That works for a dentist, but not an OB! :(

    Hey hun. I did some research for you. See if you qualify for "medical assistance". That is what your medicaid is called. I read that your access plus is for individuals who are not on a managed health care plan through medicaid. I hope this helps!
  • Oh my, thank you so, so much @mommy_three85! I'm gonna go check that out right now. I appreciate the advice & support from you all very much! I wish my case worker had informed me there were other options besides the Access Plus. But she made it seem like that was my only choice. If I can get something better & more widely accepted, I would most likely be able to be seen by our local high-risk specialist, Dr. Kouzami. And what a relief that would be! :D
  • @artistmama where in pa are you located? Have you tried planned parenthood (they are not just abortions so people who want to start issues can shut it!) The one near me (Harrisburg location ) accepts all forms of insurance and They were very nice to me, granted this was a few years ago and I just went for BC but it might be worth a shot for you.
  • You are very welcome! :) I am glad I could help because I know how frustrating it is to find quality doctors while on medicaid. Just have to research research research! :) that's what I had to do for my second pregnancy and I found the best doctor finally after an awful first pregnancy experience.
  • Maybe go to a bigger city like Pittsburgh to receive care. I'm going to Canton/Akron if I still feel like I'm receiving poor care here. I also don't like the hospital here. I'm on Medicaid and I'm willing to travel 30 min to go to a better doc and hospital for my baby.
  • @conreeaght: I'm in southwestern Pennsylvania, right outside of Johnstown, & about a 1 & 1/2 hour drive east from Pittsburgh. I actually did volunteer work with Planned Parenthood when I was in highschool many years ago, & found them to be very compassionate & informative about all pregnancy issues. I know they offer gyne services, but I never thought about obstetric care. I'll give them a call today, too! :)
  • I would go to the ER and say your cramping and something doesn't feel right. They will do an u/s. Sometimes you gotta do what you gotta do!
  • one thing you should keep in mind is many doctors won't take an ob patient after they've been seen by another doctor, and it's difficult to get anyone to see you late in pregnancy. Do you have any of those 3D ultrasound places near you?
  • @ghettobetty: The clinic I go to now is a 20 minute drive from my home, & the next closest bigger city would be Pittsburgh. Probably McGee's Women's Hospital, which is somewhere around an hour and a half from me one way. There is a great high-risk specialist who works exclusively with Conemaugh Hospital, but he doesn't accept my insurance. And there's a very good group of OB's right in my town who do accept it, but won't take on patients who are deemed high-risk. Very frustrating & unfair! :(
  • @OctMommy: I am thinking of doing that very thing, just for peace of mind, if nothing else. At least I won't go 3 months without an ultrasound, worrying that something could be going wrong with my little one.
  • @laura536: I have thought about that issue, & you're 100% correct. That's why I'm thinking an ER visit may be in my near future, & I'm going to make some calls today about applying for better insurance. We do have a 3D/4D ultrasound place here in town, with a cost of about $200. If I can scrape up the extra money, I would LOVE to have one! :)

    To this day, no one at the Clinic has ever offered me physician advice or guidance about my pregnancy. I learned what Braxton-Hicks contractions were from hearing of them on this forum & looking them up online. I didn't know I was supposed to be having urine tests & being measured at every appointment until my cousin's fiance, who just had a baby last year, said her OB did that stuff for her. Back when I had good insurance, I'd never made it past week 8 in a pregnancy, so I just really knew nothing.

    Thank you for telling me this, because I am certain the clinic will not. I will definitely monitor her fetal movements when I hit week 28.

  • @HomeBirthAdvocate: When I go to an appointment, it's the same thing every month: Weight, blood pressure, doc comes in, doesn't check my chart so I explain my medical history again & again, listens with Doppler for heartbeat, doesn't tell you if it sounds healthy or what the BPM are unless you ask, then they say, "See you in four weeks; schedule it at the check-out desk." And you're done.
    In January, I saw a resident who could not find my baby's heartbeat & could not tell me if my painful cramping was normal. I left that appointment in tears, & ended up in the ER two weeks later just so I could get answers.
    I'll definitely let you know if I manage to make any progress here in the next week or two, at most. If I truly am stuck at the Clinic, I'll begin measuring myself & testing my own urine. I'll be hitting you up for advice! :)

    I was wondering if you or anyone else can tell me what a Nuchal ultrasound is? I've seen ladies mention it in posts, & someone said it has to be done before the end of the 1st trimester. This was never offered to me, nor even mentioned. What exactly is it?

  • @HomeBirthAdvocate: I'll look for you on Facebook the next time I log in. :) Thank you for explaining that ultrasound to me. Bloodwork for genetic testing was offered, but never the Nuchal. I turned down the bloodwork for the exact same reasons you did. Finding out my child had Downs was not going to make me want to terminate my pregnancy. I'd rather be as relaxed as possible for my baby & not know, because they said the bloodwork usually yields false positives, and an amniocentesis is out of the question for me with my miscarriage risk.

    I'm glad to see I'm not the only mom who turned the genetic testing down for this same reason! :)
  • I was high risk the first time but continued to use a regular ob instead of a high-risk bc monitoring was all that could be done in my case. Is the clinic the only place u can go to or is it the only place u can go to for high risk?
